Distance Education End-of-Class Survey

Dear Student- At the beginning of the semester, you filled out a survey concerning your attitudes towards computer-aided instruction (CAI) and distance education courses. We would now like to get a sense of how you feel now that the course is over.

This information will be kept confidential (I will see it, but will not pass along any individual's responses to Professor Schlais. He will see a summary of responses)

To reply to this questionaire, please reply/extract to this E-mail. You may indicate your response to questions 1-6 by simply typing the number of the response that best represents your answer (e.g.--if you have a fair amount of experience with computer programming, just type '5' at the end of Question #1).

If you have any questions, please feel free to call or E-mail me. Thank you for your assistance.
