Although the outcomes require consideration, the core of the issue may be the need for reconsideration of the priority nursing diagnosis.  The knowledge:  health behaviors, diet, disease process outcomes are showing progress but the real issue may be actually doing the prescribed activities; further information is needed to fully determine how much he actually knows about health nutritional practices, benefits of activity/exercise, and effective stress management techniques.  Leisure participation continues to be an appropriate outcome but may fit well under another diagnosis such as altered health maintenance.
