Mr. Neuman has shown some progress towards outcomes but has not met these outcomes. 

We have done little to impact the anxiety that we originally designated as his priority nursing diagnosis.  Consider again that Mr. Neuman is sedentary, has a high fat/high caloric nutritional intake, is overweight, is not sleeping well, and has a high stress job.  Although he demonstrates the knowledge of what he is supposed to do, he has done little to change his lifestyle to accomplish the outcomes we had originally devised.  In light of this, would you consider changing the priority nursing diagnosis, the outcomes, or the interventions?
